BASIC FUNCTION
Facilitate and coordinate patient transfers as part of the operations of the UW Medicine Transfer Center and assist in the transfer process.
DISTINGUISHING CHARACTERISTICS
Under general direction, partner with Transfer Center nurses, outside hospitals and physicians, internal stakeholders (including patient flow, the emergency department, radiology, physicians, and inpatient units) to facilitate transfer of patients from outside hospitals into the UW Medicine system as well as connecting outside providers with UW Medicine on-call specialists.
TYPICAL WORK
Collaborate with multiple departments to ensure the accurate and timely flow of patient documentation, including medical record numbers, face-sheets, transfer-summaries, and imaging;
Provide information to outside hospitals and physicians regarding Transfer Center policies and procedures and guide stakeholders through our process;
Monitor program activities in relation to established Transfer Center goals, and participate in Transfer Center Quality Assurance/Quality Improvement (QA/QI) work, both by operating within expectations and participating in the planning process/feedback process;
Answer calls from outside providers in the Washington, Wyoming, Alaska, Montana and Idaho (WWAMI) region and beyond that are requesting patient transfers and consultations;
Document calls and record consult notes into Epic, including collecting demographic information;
Assist nursing staff by providing nonclinical info to callers and/or directing callers to appropriate clinics or medical center services as needed;
Attend meetings and/or conferences as program representative; and
Perform other duties as required.
MINIMUM QUALIFICATIONS
High school graduation or equivalent AND two years of experience in a healthcare or call center setting
OR
equivalent education/experience.
