Coordinate the delivery of medical/surgical care, as formulated by the medical staff, and the patient’s access to various health care services.


Coordinate patients’ entry into and through the health care system; coordinate patient care services and serve as patient advocate. Facilitate the delivery of optimal patient care.


Under general direction, use knowledge of hospital policies and procedures, insurance coverage requirements including pre-administration authorization, and third-party payor and reimbursement procedures to coordinate all patient services such as admitting, arranging diagnostic procedures, scheduling surgery and consultation requests, and scheduling services of other hospital departments to facilitate delivery of care to patients.


Make initial contact and provide information to the patient or the referring physician on behalf of a specific group of attending physicians;

Act as primary liaison among the hospital, the patient and family, referring physician, outside agencies, staff from other hospitals and the general public by providing information and advice regarding hospital services;

Coordinate inpatient and outpatient care by scheduling surgery, arranging diagnostic procedures and/or coordinating requests for consultation among other medical specialities by following established protocols;

Collect, assess, and triage information concerning the patient’s medical status from the referring physicians and the community; assess social and financial situation of patients;

Interview patient, develop evaluation of areas of concern and anticipated needs upon admission and discharge including: personal escort, transportation, housing, support care, equipment needs, transfer of records, and reappointments with physicians;

Ensure the outcome of treatment recommendations for continued care is conveyed to referring physicians;

Coordinate all necessary aspects of patient transfers to and from other hospitals;

Coordinate physician appointments for patients referred to the medical center and match patients with designated attending physician;

Receive patient complaints; obtain information and data from records and appropriate hospital staff; compose report for review and resolution, or personally resolve complaints on non-clinical issues and problems;

Act as resource and technical consultant to physicians and residents on medical/surgical services such as treatment plans, prognosis, and history of therapies;

Collect statistics, compose correspondence and record patient information in the chart; may complete insurance and disability forms; establish and distribute surgical schedules and clinic conference schedules;

Contact review organizations and insurance companies to ensure prior approval requirements are met before patients enter the hospital; present necessary medical information such as history, diagnosis, prognosis and length of stay prior to admission; provide specific medical information to Financial Services to maximize reimbursement to the hospital and physician;

May direct the work of others;

Perform related duties as required.


A Bachelor’s Degree in a medically related or public relations field AND two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology
