CLASS SERIES CONCEPT

Provide task-oriented social work services to clients/patients, families, and others.

DEFINITION

As a member of the social services team, assist Social Worker(s) in providing task-oriented social work services, such as arranging nursing home placement and ordering patient equipment for clients/patients, families, and others.

DISTINGUISHING CHARACTERISTICS

Under direct supervision, assist in providing direct task-oriented social work services to clients/patients, families, and others.

TYPICAL WORK

Assist a social worker in providing discharge planning for clients/patients and client/patient families;

Provide client/patient advocacy; act as liaison with community agencies/resources;

Provide task-oriented assessment for purpose of limited intervention;

Model task-oriented training for students and new employees; assist with educational activities provided to clients/patients and the community;

Assist clients/patients, families, and others with specific planning needs; coordinate with other staff members;

Assist in information gathering from clients/patients, groups, and agencies;

Promote optimal use of social service and health care resources;

Assist clients/patients and their families with the completion of forms, such as Medicaid, SSI, Victim's Compensation, or social service eligibility;

Participate in activities of and coordinate task-related committees of the department;

Perform related duties as required.

MINIMUM QUALIFICATIONS - UW Contract Classified

Bachelor's Degree in a behavioral science OR Associate of Arts Degree AND two years of experience in the social work field.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
