Do I need to tell my manager what my disability\/medical condition is?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseOne\" class=\"panel-collapse collapse\">\n<p>No. You are not required to share your diagnosis or medical condition with your manager\u2014and in most cases, we recommend that you don\u2019t. Your medical information should be submitted directly to the Leave and Accommodation Team or the Disability Services Office (DSO), who will keep it confidential. Some employees choose to share more about their disability as part of building trust or fostering understanding. That choice is entirely personal and should always be respected, whether you prefer to keep your health information private or speak openly about your disability.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingTwo\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseTwo\" data-toggle=\"collapse\" data-parent=\"#accordion\">2. Who reviews my request\u2014and how is it decided?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseTwo\" class=\"panel-collapse collapse\">\n<p>For <strong>staff, academic personnel and student employees<\/strong>, the Leave and Accommodation Team leads the process in collaboration with your manager and department. For <strong>faculty<\/strong>, the Disability Services Office (DSO) leads the process. These offices collaborate with you and your department to identify a reasonable accommodation based on your job duties, medical documentation and operational considerations.<\/p>\n<\/div>\n<\/div>\n<div id=\"accordion\" class=\"uwhr-accordion\">\n<div class=\"panel panel-default\">\n<div id=\"headingThree\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseThree\" data-toggle=\"collapse\" data-parent=\"#accordion\">3. Is medical documentation always required?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseThree\" class=\"panel-collapse collapse\">\n<p>In most cases, yes\u2014medical documentation is needed to help the University understand how your condition affects your work and to explore reasonable accommodations.<\/p>\n<p>That said, there are exceptions. If your disability and related work impacts are obvious (for example, you&#8217;re using a wheelchair or have a visible mobility limitation), you may not need to submit medical documentation. What constitutes a medically necessary accommodation?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseFour\" class=\"panel-collapse collapse\">\n<p>Accommodations should be supported by the medical documentation and should predictably mitigate the limitations\/restrictions, allowing the employee to perform all of the essential functions of the job.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingFive\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseFive\" data-toggle=\"collapse\" data-parent=\"#accordion\">5. Do I have to use a specific form to request an accommodation?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseFive\" class=\"panel-collapse collapse\">\n<p>To avoid delays, we encourage you to have your provider complete the <a class=\"uwhr-form  \" target=\"_blank\" href=\"https:\/\/hr.uw.edu\/dso\/wp-content\/uploads\/sites\/6\/2025\/06\/health-care-provider-statement-employee-20250609-a11y.pdf\">Health Care Provider Statement (PDF)<\/a> or <a class=\"uwhr-form  \" target=\"_blank\" href=\"https:\/\/hr.uw.edu\/dso\/wp-content\/uploads\/sites\/6\/2016\/07\/health-care-provider-statement-employee-parking-20250515-a11y.pdf\">Health Care Provider Statement - Parking (PDF)<\/a> if your request is limited to parking or transportation. While not required, these forms help ensure we receive the right level of detail from the start. If you choose not to use the forms\u2014or if what we receive is incomplete\u2014we may follow up for clarification, which could delay the process.<\/p>\n<\/div>\n<\/div>\n<div id=\"accordion\" class=\"uwhr-accordion\">\n<div class=\"panel panel-default\">\n<div id=\"headingSix\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseSix\" data-toggle=\"collapse\" data-parent=\"#accordion\">6. What does \u201creasonable accommodation\u201d mean?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseSix\" class=\"panel-collapse collapse\">\n<p>A reasonable accommodation is a change to how work is done or the work environment provided it doesn\u2019t cause an undue hardship for the university. Examples might include flexible schedules, modifying processes, assistive equipment, communication supports, or temporary changes in job duties. The best solution is one that meets your needs and supports the work of your team.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingSeven\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseSeven\" data-toggle=\"collapse\" data-parent=\"#accordion\">7. Can I bring someone with me to an accommodation meeting for support?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseSeven\" class=\"panel-collapse collapse\">\n<p>In some cases, employees may request to bring a support person to provide quiet emotional support during an accommodation meeting. Support persons are not there to speak on your behalf but may offer reassurance or assist with notetaking.<\/p>\n<p>Because this is not a routine part of the process, advance approval is required. To learn more, including who may serve in this role and how to request approval, see <a href=\"https:\/\/hr.uw.edu\/dso\/services\/faculty-and-staff\/\">Emotional Support Persons in Disability Accommodation Meetings<\/a>.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingEight\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseEight\" data-toggle=\"collapse\" data-parent=\"#accordion\">8. What if my condition changes &#8212; or I need something different later?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseEight\" class=\"panel-collapse collapse\">\n<p>You can request an update or review of your accommodation at any time. In fact, accommodations should be reviewed when your job duties or medical condition changes or if your healthcare provider indicates that your condition is temporary. Your accommodations can change with you and your position.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingNine\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseNine\" data-toggle=\"collapse\" data-parent=\"#accordion\">9. How long does the process take?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseNine\" class=\"panel-collapse collapse\">\n<p>Timelines vary depending on the complexity of your request and the responsiveness of all parties involved (including your healthcare provider). In general, you should expect initial follow-up within a few business days of submitting your request. If you experience a delay, reach out to the Leave &amp; Accommodation Team or DSO to check on your request status.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingTen\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseTen\" data-toggle=\"collapse\" data-parent=\"#accordion\">10. Can I be denied an accommodation?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseTen\" class=\"panel-collapse collapse\">\n<p>You may not always receive the specific accommodation you request. If another option is available that meets your needs and is reasonable for the role, an <strong>equally effective alternative<\/strong> may be offered instead.<\/p>\n<p>If no reasonable accommodation can be identified that allows you to perform the essential functions of your job, a <strong>protected leave of absence<\/strong> may be offered as an alternative.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingEleven\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseEleven\" data-toggle=\"collapse\" data-parent=\"#accordion\">11. What if I think the process is unfair or burdensome?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseEleven\" class=\"panel-collapse collapse\">\n<p>We are committed to ensuring the accommodation process is fair, respectful, and accessible. Can I request a remote or hybrid work arrangement as an accommodation?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseTwelve\" class=\"panel-collapse collapse\">\n<p>Possibly. Remote or hybrid work can be considered as a reasonable accommodation depending on your job duties, the nature of your medical needs, and the operational needs of your unit. The University evaluates each request individually.<\/p>\n<\/div>\n<\/div>\n<div class=\"panel panel-default\">\n<div id=\"headingThirteen\" class=\"panel-heading\">\n<h5 class=\"panel-title\"><a class=\"panel-link collapsed\" href=\"#collapseThirteen\" data-toggle=\"collapse\" data-parent=\"#accordion\">13. I\u2019m a manager and my employee provided a doctor&#8217;s note requesting workplace accommodation, what do I do?<i class=\"fa panel-title-icon\"><\/i><\/a><\/h5>\n<\/div>\n<div id=\"collapseThirteen\" class=\"panel-collapse collapse\">\n<p>Reach out to your Leave and Accommodation Specialist or DSO to start the interactive process. Do not accept the doctor\u2019s note.
