{"id":398,"date":"2018-02-13T13:22:12","date_gmt":"2018-02-13T21:22:12","guid":{"rendered":"http:\/\/hr.uw.edu\/dso\/?page_id=398"},"modified":"2024-11-04T15:15:13","modified_gmt":"2024-11-04T23:15:13","slug":"employees","status":"publish","type":"page","link":"https:\/\/hr.uw.edu\/dso\/disability-parking\/employees\/","title":{"rendered":"Disability parking for employees"},"content":{"rendered":"<h4 id=\"uw-seattle-uw-tacoma-or-harborview-medical-center\">UW Seattle, UW Tacoma, or Harborview Medical Center<\/h4>\n<p><strong>If you already have a WA State Disability Parking Permit<\/strong> and need either disability parking or dial-a-ride services, you will need to provide the Disability Services Office with the following:<\/p>\n<ul>\n<li>A photocopy of your WA state DOL disability parking placard<\/li>\n<li>A photocopy of your wallet card<\/li>\n<li><a class=\"uwhr-form  \" target=\"_blank\" href=\"https:\/\/hr.uw.edu\/dso\/wp-content\/uploads\/sites\/6\/2016\/07\/disability-accommodation-request-DAR-employee-20250502-a11y.pdf\">Disability Accommodation Request (PDF)<\/a>, to be filled out by you<\/li>\n<\/ul>\n<p><strong>If you DO NOT have a WA State Disability Parking Permit<\/strong>, the following is required:<\/p>\n<ul>\n<li><a class=\"uwhr-form  \" target=\"_blank\" href=\"https:\/\/hr.uw.edu\/dso\/wp-content\/uploads\/sites\/6\/2016\/07\/health-care-provider-statement-employee-parking-20250515-a11y.pdf\">Health Care Provider Statement - Parking (PDF)<\/a>, to be filled out and signed by your health care provider<\/li>\n<li><a class=\"uwhr-form  \" target=\"_blank\" href=\"https:\/\/hr.uw.edu\/dso\/wp-content\/uploads\/sites\/6\/2016\/07\/disability-accommodation-request-DAR-employee-20250502-a11y.pdf\">Disability Accommodation Request (PDF)<\/a>, to be filled out by you<\/li>\n<\/ul>\n<p>Send the required documents to the Disability Services Office as email attachments to <a href=\"mailto:dso@uw.edu?subject=Disability parking\">dso@uw.edu<\/a> or by fax: 206-685-7264.<\/p>\n<p class=\"uwhr-toc-top-btn\"><a href=\"#toc\">Return to top<\/a><\/p><h4 id=\"uw-bothell\">UW Bothell<\/h4>\n<ul>\n<li><a href=\"https:\/\/www.uwb.edu\/commuter-services\/parking\/disability-parking\">UW Bothell disability parking information<\/a><\/li>\n<\/ul>\n<p class=\"uwhr-toc-top-btn\"><a href=\"#toc\">Return to top<\/a><\/p><h4 id=\"temporary-permits\">Temporary permits<\/h4>\n<p>While UW Seattle employees are in the process of submitting documentation to the Disability Services Office, UW Transportation Services will provide six weeks of disability parking.<\/p>\n<p>Contact Transportation Services at 206-221-3701 or <a href=\"mailto:ucommute@uw.edu\">ucommute@uw.edu<\/a> to obtain this temporary permit.<\/p>\n<p class=\"uwhr-toc-top-btn\"><a href=\"#toc\">Return to top<\/a><\/p><h4 id=\"limited-timeframe-for-all-permits\">Limited timeframe for all permits<\/h4>\n<p>Disability parking is authorized for a specific timeframe only.
