{"id":6140,"date":"2024-05-10T15:22:56","date_gmt":"2024-05-10T22:22:56","guid":{"rendered":"https:\/\/hr.uw.edu\/comp\/?page_id=6140"},"modified":"2025-05-22T14:18:56","modified_gmt":"2025-05-22T21:18:56","slug":"patient-care-coordinator-supv","status":"publish","type":"page","link":"https:\/\/hr.uw.edu\/comp\/classified-staff\/patient-care-coordinator-supv\/","title":{"rendered":"Patient Care Coordinator Supv"},"content":{"rendered":"<p><strong>UW CODE AND REPRESENTATIVE GROUP<\/strong><\/p>\n<p>18069 (NE S SEIU 925 Supv)<br \/>\n22483 (NE H NI SEIU 925 Supv)<\/p>\n<p><strong>BASIC FUNCTION<\/strong><\/p>\n<p>Supervise Patient Care Coordinators and\/or other assigned staff and coordinate patient&#8217;s entry into and through the health care system.<\/p>\n<p><strong>DISTINGUISHING CHARACTERISTICS<\/strong><\/p>\n<p>With delegated authority, interview and recommend selection of applicants, conduct training, assign and schedule work, act upon leave requests, conduct annual performance evaluations and recommend disciplinary action.<\/p>\n<p>Under administrative direction, coordinate the delivery of medical\/surgical care for patients in assigned specialty areas.<\/p>\n<p><strong>TYPICAL WORK<\/strong><\/p>\n<p>Supervise assigned staff ensuring compliance with regulatory agencies, insurance and other financial requirements, and hospital policies and procedures;<\/p>\n<p>Assist staff in resolving patient care problems;<\/p>\n<p>Make initial contact and provide information to the patient or the referring physician on behalf of a specific group of attending physicians;<\/p>\n<p>Act as primary liaison among the hospital, the patient and family, referring physician, outside agencies, staff from other hospitals and the general public by providing information and advice regarding hospital services;<\/p>\n<p>Coordinate inpatient and outpatient care by scheduling surgery, arranging diagnostic procedures and\/or coordinating request for consultation among other medical specialities by following established protocols;<\/p>\n<p>Collect, assess, and triage information concerning the patient&#8217;s medical status from the referring physicians and the community; assess social and financial situation of patients;<\/p>\n<p>Interview patient; develop evaluation of areas of concern and anticipated needs upon admission and discharge including: personal escort, transportation, housing, support care, equipment needs, transfer of records, and reappointments with physicians;<\/p>\n<p>Ensure the outcome of treatment recommendations for continued care is conveyed to referring physicians;<\/p>\n<p>Coordinate all necessary aspects of patient transfers to and from other hospitals;<\/p>\n<p>Receive patient complaints; obtain information and data from records and appropriate hospital staff; compose report for review and resolution, or personally resole complaints on non-clinical issues and problems;<\/p>\n<p>Act as resource and technical consultant to physicians and residents on medical\/surgical services;<\/p>\n<p>Perform the work of Patient Care Coordinator;<\/p>\n<p>Perform related duties as required.<\/p>\n<p><strong>MINIMUM QUALIFICATIONS<\/strong><\/p>\n<p>A Bachelor&#8217;s Degree in a medically related or public relations field AND two years of experience as a Patient Care Coordinator.<\/p>\n<p>Equivalent education and\/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and\/or registration.<\/p>\n<p><strong>CLASSIFICATION HISTORY<\/strong><\/p>\n<p>06\/07\/1991: New Class<br \/>\n06\/01\/2005: CSR Class Conversion<\/p>\n","protected":false},"excerpt":{"rendered":"<p>UW CODE AND REPRESENTATIVE GROUP 18069 (NE S SEIU 925 Supv) 22483 (NE H NI SEIU 925 Supv) BASIC FUNCTION Supervise Patient Care Coordinators and\/or other assigned staff and coordinate [&hellip;]<\/p>\n","protected":false},"author":49,"featured_media":0,"parent":12,"menu_order":0,"comment_status":"closed","ping_status":"closed","template":"","meta":{"_acf_changed":false,"_lmt_disableupdate":"no","_lmt_disable":"no","footnotes":""},"class_list":["post-6140","page","type-page","status-publish","format-standard","hentry"],"acf":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.2 -
