BASIC FUNCTION<\/strong><\/p>\n

Performs professional pharmaceutical dispensing functions in a pharmacy and\/or performs clinical functions within the patient environment.<\/p>\n

DISTINGUISHING CHARACTERISTICS<\/strong><\/p>\n

Prepares and dispenses medications; provides medication information\/education to nurses, physicians\/clinicians and patients; instructs pharmacy and veterinary students, residents, interns and\/or assistants; and provides pharmaceutical care to patients.<\/p>\n

TYPICAL WORK<\/strong><\/p>\n

Provides medication information to patients, physicians, nurses and allied healthcare providers;<\/p>\n

Conducts a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical including monitoring individual patient profiles for medication duplications, interactions, allergies, appropriate dose, strength and route of administration at the time each order is received; documents relevant information in the patient medication profile or chart;<\/p>\n

Evaluates prescriber\u2019s orders and verifies the data entered into the patient medication record;<\/p>\n

Interviews patients and provides education for the self-administration of take-home medications;<\/p>\n

Operates hospital pharmacy, retail, or outpatient pharmacy; compounds, labels and dispenses medications and legend devices;<\/p>\n

Ensures delivery of pharmaceutical to patients in a timely manner;<\/p>\n

Assists with the instruction of students, residents, and staff pharmacists regarding clinical pharmacy, distribution and medication information;<\/p>\n

Assists with periodic peer review and evaluation;<\/p>\n

Participates in the implementation of formulary management, optimizing patient care through appropriate selection of treatment options;<\/p>\n

Prepares estimates; requisitions, receives, and stores medications, pharmaceuticals, instruments, dressings, apparatus, and surgical supplies;<\/p>\n

Keeps records of alcohol and narcotics received, on hand, dispensed, and on order; maintains perpetual inventory of all medications and surgical supplies; prepares periodic reports for attending physicians and supervisors;<\/p>\n

Keeps medical supply room and pharmacy secure, orderly, and in sanitary condition;<\/p>\n

Prepares and standardizes solutions and other preparations for use in research work conducted by members of medical and dental staff;<\/p>\n

Performs related duties as required;<\/p>\n

Directs the work of pharmacy technicians, pharmacy assistants, interns, pharmacy students and\/or residents.<\/p>\n

LEGAL REQUIREMENT(S)<\/strong><\/p>\n

A valid Washington State license to practice pharmacy or eligibility to obtain license prior to employment.<\/p>\n

MINIMUM QUALIFICATIONS<\/strong><\/p>\n

One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.<\/p>\n

Equivalent education and\/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and\/or registration.<\/p>\n
