Evidence of Insurability may be required before reinstatement.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Enroll\"><\/span>Enroll<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>To continue your coverage through self-pay:<\/p>\n<ol>\n<li>Fill out the <a href=\"https:\/\/www.hca.wa.gov\/assets\/pebb\/50-0135-pebb-continuation-coverage-unpaid-leave-election-change-2026.pdf\">Leave Without Pay Election\/Change 2026<\/a> form<\/li>\n<li>Write a check in the amount of your first month&#8217;s premiums (<a href=\"https:\/\/www.hca.wa.gov\/assets\/pebb\/50-0300-pebb-continuation-coverage-monthly-premiums-2026.pdf\">PEBB Continuation Coverage Monthly Premiums 2026<\/a>)<\/li>\n<li>Complete the <a href=\"https:\/\/www.hca.wa.gov\/assets\/pebb\/42-0450-pebb-electronic-debit-service.pdf\">Electronic Debit Service Agreement<\/a> (optional)<\/li>\n<li>Mail the completed forms and check to the Health Care Authority (address listed on the form)<\/li>\n<\/ol>\n<h2><span class=\"ez-toc-section\" id=\"Cost\"><\/span>Cost<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>You choose what coverage you want to continue through self-pay. Coverage is offered by the state Public Employees Benefits Board (PEBB):<\/p>\n<ul>\n<li><a href=\"https:\/\/www.hca.wa.gov\/assets\/pebb\/50-0300-pebb-continuation-coverage-monthly-premiums-2026.pdf\">Monthly premiums for medical and dental plans 2026<\/a><\/li>\n<li><a href=\"https:\/\/www.hca.wa.gov\/employee-retiree-benefits\/pebb-continuation-coverage\/life-insurance-premiums-unpaid-leave-only\" target=\"_blank\" rel=\"noopener noreferrer\">Monthly premiums for life insurance<\/a><\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Long-term-disability\"><\/span>Long-term disability<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Long-term disability (LTD) insurance is typically not in force while you\u2019re on an unpaid leave of absence.<\/p>\n<p>However, if you are on an approved educational leave without pay, you can elect to continue your long-term disability (LTD) insurance on a self-paid basis. When your educational leave is partially paid, deductions are taken from your salary, and you can elect to self-pay the difference. Contact Benefits if you\u2019d like to self-pay the difference.<\/p>\n<p>If you\u2019re on unpaid leave that\u2019s not approved as educational, you can\u2019t self-pay to continue LTD insurance, but you can reinstate coverage by applying within 10 days of returning from your leave without pay.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"FMLA-leave\"><\/span>FMLA leave<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If your leave is fully or partially covered by the Family and Medical Leave Act (FMLA), some of your coverages may not require that you self-pay for insurance coverage.<\/p>\n<p>Contact your <a href=\"https:\/\/hr.uw.edu\/benefits\/contact-us\/hr-consultant-list\/\" target=\"_blank\" rel=\"noopener\">HR consultant<\/a> if you have questions about whether a leave may be covered by the FMLA.<\/p>\n<p>If you receive written notification that a leave of absence without pay is covered by FMLA, you should email Benefits for information about benefits eligibility under FMLA.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Active-military-duty-leave\"><\/span>Active military duty leave<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If you are called to active-duty military service, special rules apply for continuation of your medical, dental, vision, life and long-term disability insurance benefits. Please work with your Leave Specialist to ensure you are placed on your appropriate military leave. Contact <a href=\"mailto:benefits@uw.edu\">UW Benefits<\/a> if you have questions about your benefit eligibility while on military leave.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"If-you-dont-continue-your-coverage\"><\/span>If you don\u2019t continue your coverage<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If you choose not to continue your benefits on a self-paid basis:<\/p>\n<ul>\n<li>Your medical, vision and dental insurance will end as of the last day of the month in which you earned at least eight hours\u2019 pay. Your medical, vision and dental insurance will not be reinstated until the first day of the month in which you are in pay status.<\/li>\n<li>Your life insurance will end as of the last day of the month in which the full monthly premium was paid. On return from leave or layoff, no optional coverage is reinstated unless applied for and approved by the insurance company.<\/li>\n<li>Your long-term disability (LTD) insurance will end as of the last day in pay status and will be reinstated on return as long as the enrollment form is completed within ten (10) days of return from leave.
