{"id":4393,"date":"2017-11-23T12:05:46","date_gmt":"2017-11-23T20:05:46","guid":{"rendered":"http:\/\/hr.uw.edu\/benefits\/?page_id=4393"},"modified":"2026-02-04T08:28:44","modified_gmt":"2026-02-04T16:28:44","slug":"gaip-self-pay-option","status":"publish","type":"page","link":"https:\/\/hr.uw.edu\/benefits\/insurance\/health\/graduate-appointees\/gaip-self-pay-option\/","title":{"rendered":"GAIP: Self-pay option"},"content":{"rendered":"<p>In some situations, you may be eligible\u00a0to receive GAIP health coverage by paying for it yourself. The self-pay option allows you to either continue your existing coverage or get coverage when your position doesn\u2019t provide UW-paid coverage. Outside of loss of UW-paid coverage during the plan year (described below), the UW Benefits office must approve your eligibility for the self-pay option.<br \/>\nWhile on self-pay, if you&#8217;re not registered for classes and do not pay the student activity fee, your benefits at Hall Health will be paid at the network level of benefits.<\/p>\n<h4>Eligibility<\/h4>\n<p>You may be eligible for the self-pay GAIP option if:<\/p>\n<ul>\n<li>You lose eligibility for UW-paid GAIP coverage<\/li>\n<li>An outside source pays for your position<\/li>\n<li>You\u2019re on approved academic leave<\/li>\n<\/ul>\n<h5>Loss of UW-paid coverage<\/h5>\n<p>If your job is ending, you can continue your GAIP coverage using the self-pay option. Once you enroll, your coverage will continue through the end of the plan year, which ends on September 30.<\/p>\n<p>LifeWise will automatically send you a self-pay election notice once they receive notice that your position\u00a0is ending. This notice includes a deadline to respond.<\/p>\n<h5>Appointment paid by an outside source<\/h5>\n<p>If you\u2019re paid by an outside source (UW doesn\u2019t administer your paycheck), you\u2019re eligible for the self-pay option if you:<\/p>\n<ul>\n<li>Have a position in an eligible job class, AND<\/li>\n<li>Receive paid-direct funding at $800 per month or greater for at least one academic quarter, AND<\/li>\n<li>Are enrolled in at least 10 credits for that same quarter.<\/li>\n<\/ul>\n<p>Contact your department coordinator if you would like to enroll. Your department must request the self-pay option on your behalf. You may enroll during the quarter your position was established and continue your coverage for the rest of the plan year, which ends September 30.<\/p>\n<p>Note that Howard Hughes Medical Institute (HHMI) fellows are eligible to participate in the self-pay program, provided the same procedures (above) are followed.<\/p>\n<h5>Academic leave<\/h5>\n<p>You can use the self-pay option during academic leave as long as you are in a GAIP-eligible position before your leave, and you\u2019ll be returning to a GAIP-eligible position after your leave.<\/p>\n<p>After your department approves your academic leave request, send the following documents to the UW Benefits office:<\/p>\n<p style=\"padding-left: 40px\"><a class=\"uwhr-form  \" target=\"_blank\" href=\"\">GAIP - Petition for Self-Pay Graduate Appointee Insurance While on Academic Leave <\/a><\/p>\n<p style=\"padding-left: 40px\">A copy of your graduate leave approval confirmation which you will receive after your leave request is approved by your graduate program and you have paid the graduate leave fee.<\/p>\n<p>International Student leave requests will also have an additional approval requirement from the International Student Services Office.<\/p>\n<p>See GAIP\u00a0<a href=\"http:\/\/hr.uw.edu\/benefits\/health-insurance\/graduate-appointees-health-insurance\/gaip-leave-of-absence\/\">leave of absence<\/a> for information regarding continuation of coverage while on an approved ASE contract leave of absence.<\/p>\n<h4>Self-pay premiums<\/h4>\n<p>Once enrolled in the self-pay option, you pay the monthly premium for yourself and any dependents.
